University Behavioral Health offers free mental health and chemical dependency assessment and referral services for children, adolescents, adults, and seniors. If treatment is advised, we can assist with admission to a UBH program or recommend other treatment options in the community.

The Care Center clinical staff is available 24 hours a day, seven days a week, to respond to calls. Private and confidential assessments and referral information are provided at no charge.

Patients can be referred to UBH in several ways; outpatient therapists, primary care physicians, mental health providers, emergency services clinicians, doctors, other inpatient hospitals, case managers, employee assistance programs, family members, community agencies, hospitals, clergy, friends and patients themselves.

When calling the UBH Care Center to refer someone for a free assessment or hospital admission, please provide the following information so that we can best serve the patient.

• Your name, relationship to the patient, and phone number.
   
• A brief description of the patient's problem and why you are making the referral.
   
• Patient's name, date of birth, social security number, address, and phone number.
   
• Insurance company, insured's name, policy number, company phone number (if available)
   
• Any special instructions regarding treatment program, doctor preference, release of information or follow-up with you if you are a clinician.
   
• What time we can schedule the patient to be seen.

The Care Center works rapidly to complete the admission process; however, it may take longer depending upon the insurance company's requirements.

If you refer a patient to our program, you can be assured that you will receive individualized communication from the treatment team to ensure full coordination of care, as well as:

• A responsive referral process that you initiate with a single telephone call
   
• An interim and final report regarding your patient's diagnosis and individual treatment plan
   
• A comprehensive medication assessment for your patient, as well as clinical management of patient comorbid conditions
   
• A team consisting of a psychiatrist, social worker, and psychiatric nurse
   
• Integrated follow-up with providers to coordinate the highest level of care
